How to appeal
This guidance provides information about the how to appeal and about the appeals procedure in respect of all the jurisdictions of the Tribunal. The Guidance can be downloaded and printed. To obtain a hard copy of the Guidance contact the Care Standards Tribunal Secretariat.
Appeal application forms (to be completed by appellants)
Form A
Use this form if you want to appeal against:
- your name being on the Protection of Children Act list and
- your name being on the Protection of Vulnerable Adults list and
- being restricted or stopped from working with children in schools, colleges and universities.
Form B
Use this form if you want to appeal against:
- the Commission for Social Care Inspection (CSCI) or decision of the Health Care Commission;
- the Chief Inspector of Schools in England (Ofsted) or a decision of the body responsible for the approval of home child care providers;
- the Welsh Ministers; (except for decisions to suspend the registration of child minders/day care providers);
- the General Social Care Council (GSCC) or Care Council Wales;
- the Secretary of State about the registration of an independent school.
- the Order made by a justice of the peace about the registration of establishments and agencies; the registration of childminders and day care providers; the approval of Home Child Care Providers.
Form C
Appeals against decision of the Chief Inspector of Schools or the Welsh Ministers in respect of the suspension of registration of child minders, day care providers or approval of child home care providers.

Use this response form to appeal against:
- inclusion on Protection of Children Act List;
- restriction or prohibition from working in schools;
- provisional inclusion on Protection of Children Act List
Form B
Use this response form to appeal against:
- the Commission for Social Care Inspection (CSCI) or decision of the Health Care Commission;
- the Chief Inspector of Schools in England (Ofsted) or a decision of the body responsible for the approval of home child care providers;
- the Welsh Ministers; (except for decisions to suspend the registration of child minders/day care providers);
